Summary:

Plate osteosynthesis is recognized as a reliable method for the treatment of intra-articular fractures of the distal femur. To achieve anatomic reduction and stabilization of complex intra-articular fractures during plating, extended approaches have been developed. These exposures provide for excellent visualization of the articular and metaphyseal fracture fragments and have been used in combination with traditional AO fixation techniques. However, they are associated with high rates of complications and the need for autogenous bone grafting. The purpose of this article is to review the extended approaches to the distal femur and the reported clinical results associated with their use.
