Transconjunctival sclerotomies are made in the standard fashion, and vitrectomy is performed. After the surgery, the cannulas are removed by holding the silicon piece and no sutures are required.

Interface vitrectomy is used after a full vitreous cavity air fill for enhanced visualization of the vitreous base. The microvitrector is placed within the vitreous base with the port directed away from the retinal surface, and the vitreous removed. Video demonstrates vitreous base truncation under air.

SST-IOL technique: Given the risk of anisometropic amblyopia, a second 3-piece IOL was placed in the right eye as a piggyback lens in SST-IOL fashion. A silicone IOL (Bausch & Lomb LI61AO, Bridgewater, NJ) was used to decrease IOL interface opacification or fibrosis risk. The same technique as outlined above was used except that the ciliary sulcus-based sclerotomies were placed at 3- and 9-o' clock and were made 1 mm, not 1.5 mm, from the limbus. This is the first report of SST-IOL placed in piggyback fashion to our knowledge.

This is a patient with Coats’ disease and the transcleral diode laser was performed. Laser photocoagulation was administered transcleraly with the DioPexy retinopexy probe (IRIS DioPexy, IRIS Medical Instru-ments, Mountain View, CA, USA) under retrobulbar and topical anesthesia and observed by using a volk 30-D lens and surgical microscope with f=300 objective lens. We can see the yellow subretinal exuates and the tortuous vessles clearly.
