How will YOU Face the Case?

To engage readers around plastic surgery case reports from around the globe and enact the mission of PRS Global Open, we challenge surgeons and residents to "Face the Case" on Facebook, Twitter, and Instagram.

First, we post video of one of our international Resident Ambassadors presenting the patient and problems from a PRS Global Open case report. They then present two treatment options. Using comments on Instagram and Facebook and polls on Twitter and Instagram Stories, readers vote on how they’d proceed.

Following the vote, we post video of an Associate Editor or other global plasic surgery expert discussing the poll results, how they’d treat the patient, and what the authors ultimately did. Our International hosts and guests help us tap into the global plastic surgery network on social media.

A final video combining the presentation, polling, and analysis links to the paper; a fitting conclusion to this interactive, multi-media campaign utilizing social media to encourage engagement and discussion around plastic surgery case reports, which in helps us meet our mission statement: improving plastic surgery education article-by-article. A collection of these wrap-up videos are paired with their respective case reports in this collection.

In this #FacetheCase video, PRS Global Open Resident Ambassador Dr. Macarena Vizcay presents a 31-year-old woman with a history of prior resection of endometrial tissue around her Pfannenstiel incision presented 5 years later with recurrence of pain and a mass associated with menses despite maximal medical treatment for endometriosis. Computed tomography showed an infiltrative soft tissue mass. En bloc resection of the mass and total abdominal hysterectomy and bilateral salpingo-oophorectomy were performed.

In this #FacetheCase video, PRS Global Open Resident Ambassador Dr. Murilo Secanho presents a 28-year-old, healthy man who was involved in an all-terrain vehicle accident. The accident resulted in a glabrous defect measuring 7.5 cm × 7.2 cm. He underwent urgent carpal tunnel and hand compartment releases, washout, and debridement. Avulsion of the thenar muscles from their proximal origins and abrasion through the transverse carpal ligament were noted. The median nerve was in continuity, but 50% of the nerve diameter was disrupted at the carpal tunnel; the surrounding 7 cm of nerve had a purple, bruised appearance

The problem list included

  • Defect of glabrous skin measuring 7.5 X 7.2 cm
  • Avulsion of the thenar muscles from their proximal origins
  • Four large fascicular groups of median nerve disrupted, including the third webspace sensory branch.

Physical examination revealed a left temporoparietal skull depression with marked asymmetry in head shape, a low-set left ear, and a large posterior fontanelle. A 3D CT Scan demonstrated synostosis of the left pterion which involved the coronal, frontosphenoidal, sphenoparietal, and temporoparietal sutures.

The patient was morbidly obese with BMI 55 kg/m2, with very thick subcutaneous tissues in legs and thighs. She previously enrolled in a weight loss program for 6 months, has undergone diet modification, compression therapy and exercise, but has seen no significant improvement.

In this #FacetheCase video, PRS Global Open Resident Ambassador Dr. Mohamad Rachadian Ramadan presents a 79-year-old patient was referred for further management of an advanced malignant melanoma. He had history of excision biopsy, and histology revealed incomplete excision of a pT2a malignant melanoma with a Breslow thickness of 1.3 mm, a mitotic rate of 0 mm2, and no ulceration. On admission for surgery, right infraorbital cheek swelling was noted, which was highly suspicious of in-transit metastasis, the planned excision was therefore extended to include the infraorbital area. After wide local excision, the defect included the entire right nasal sidewall, medial two thirds of lower eyelid anterior lamella, and most of the infraorbital cheek including the zygomatic muscles.
