Carrie Graham, PhD, is an assistant adjunct professor, Department of Social & Behavioral Sciences, University of California, San Francisco, and a doctorate in medical sociology. She is on the research staff at the Center for Community Wellness at the University of California, Berkeley. Her current work is a study of caregiver needs and issues around transitioning care recipients from hospital to home care.
Linda Anderson, MSW, is division manager of Case Management Services, Contra Costa County Aging and Adult Services, Martinez, California. Among her responsibilities are Adult Protective Service, Multipurpose Senior Service Program (a Medicaid Home & Community Based Care waiver program), and Information and Assistance/Referral/Screening Unit. She is also in Bureau of Aging & Adult Services.
Robert Newcomer, PhD, is a professor of medical sociology, Department of Social & Behavioral Sciences, University of California, San Francisco. A major interest is the coordination between acute and chronic care systems and the contribution of community services to the quality of healthcare outcomes. He has published a number of articles involving case management interventions in managed care and among special populations.
