This video shows planning of a keystone reconstruction following resection of a large anterior thigh sarcoma defect, which was previously irradiated. Flaps were designed, centred over dominant perforators, which were identified using handheld Doppler. Progressive deep fascial incision was carried out and tension on wound closure was assessed- a second flap was designed as a backup to assist in closure if required.
