The biologic effect on facial soft tissue following the injection of Hyaluronic Acid (HA) fillers are still being investigated, but the clinical effects suggest more than just volume replacement. Reconstituting HA fillers at low concentrations utilizing lidocaine allows these injectable agents to be placed superficially with fine gauge needles to improve the appearance of fine lines. The long-term effects of reconstituted HA fillers on fine lines clinically suggests both a volumetric as well as a regenerative biologic effect, as wrinkle ablation in many patients is apparent months to years following injection. This video discusses both the technique and efficacy of utilizing reconstituted low concentration HA fillers for facial rejuvenation.

As we gain more experience with Hyaluronic Acid (HA) fillers, their effects beyond volume correction are becoming recognized and the effects of HA fillers are not limited to static changes in facial shape. By specific placement along the muscular origin or insertion, HA fillers can also produce dynamic changes in animation, restoring facial balance. This video discusses both the technique and efficacy of utilizing HA fillers to effect dynamic animation changes without the use of neuromodulators.

Read the urgent safety advisory issued by the Multi-Society Task Force for Safety in Gluteal Fat Grafting for crucial information on the procedure.
The mortality rate from gluteal fat grafting is unacceptable. Despite efforts at defining “safety zones” and “angles” of cannula direction, the crux of the problem appears to be the depth of cannula penetration and not the angle of insertion nor the zone of the gluteus muscle injected.  Dynamic cadaver studies demonstrate that fat placed just beneath the gluteus maximus fascia will migrate deep to the sub muscular space with the potential for venous tear and pulmonary fat embolism. A call is hereby made for a halt to fat grafting beneath the fascia of the gluteus muscle.

In this video the MACS-lift is presented as the cornerstone of our current concept of comprehensive facial rejuvenation. It effectively and safely treats the aging of the lower half of the face and the upper neck in the majority of aging men and women. The addition of microfat grafting to treat the age-related centrofacial deflation adds to the power and longevity of the rejuvenation, and also allows treatment of non-age-related features such as the tear trough and the nasolabial fold. To treat the category of patients with a more demanding neck laxity, platysmal bands and submental fat deposition we can add a subplatysmal fat resection via a submental incision, and/or a posterior subplatysmal neck plasty with posterior platysma suspension. All of these techniques are included in detail in this video, together with some theoretical considerations.

This video demonstrates the technique of combining upper and lower blepharoplasty with endoscopic brow lifting. Part 1 of 3.

The upper lid blepharoplasty preserves upper lid orbicularis oculi to maintain upper lid volume, while the lower blepharoplasty utilizes a transconjunctival approach to the lower lid fat in combination with a formal skin flap dissection to address concerns regarding treating lower eyelid skin. Shaping of the lower lid utilizing a lateral retinacular canthopexy as well as resuspension of the lower lid orbicularis oculi is demonstrated. The endoscopic brow lift is preformed via an optical cavity formed via a subperiosteal dissection accessed through a small incision in the lateral upper lid, followed by traditional subperiosteal endoscopic release of the forehead and temporal region. Brow elevation, accentuation of brow arching and brow fixation are performed utilizing an Endotine fixation device.

Lipoabdominoplasty combines safety, efficacy and aesthetic control when contouring the female silhouette. Preoperative markings are performed with the patient upright and is fundamental to achieving a desired result. The technical details of abdominoplasty include Scarpa fascia preservation of the lower abdomen to aid in healing and lessen seroma formation. From an aesthetic perspective, we believe that a natural soft contour of the abdomen is an important aesthetic goal, rather than a "high definition" defined by an extreme unnatural appearance. For this reason, when we preform liposuction of the abdomen and truck, we tend to be conservative in fat reduction to maintain the natural volume associated with a mature female shape. Our main objective remains obtaining consistent results while minimizing the possibility of complications when combining liposuction with abdominoplasty.

Part 1 of 2. Short scar neck lifting is a procedure that focuses on restoring cervical contour, and is indicated in younger patients who present with platysma banding not associated with facial laxity. The procedure is performed through a postauricular incision in combination with a submental incision, and involves platysmaplasty, the precise contouring of cervical fat, as well as excision of excess cervical skin. Lipoabdomonoplasty has been advocated by Avelar and Saldanha, a technique which has modernized the conventional abdominoplasty that saw few modifications in the last 50 years. Lipoabdominoplasty is based on three basic points: a) dissection through liposuction; b) preservation of Scarpa’s fascia and deep fat of lower abdomen; and c) narrow undermining on the midline for plication.

A large number of patients were not good candidates for the lipoabdominoplasty protocol, because many of them have mild skin flaccidity in the abdomen, partial rectus diastasis and high position of belly button. For these patients, Novaes and Ribeiro, created a new classification, as shown in this video. This personal evolution can reduce flap necrosis, seromas, and hematomas, with faster recovery time.
