Read the urgent safety advisory issued by the Multi-Society Task Force for Safety in Gluteal Fat Grafting for crucial information on the procedure.
The mortality rate from gluteal fat grafting is unacceptable. Despite efforts at defining “safety zones” and “angles” of cannula direction, the crux of the problem appears to be the depth of cannula penetration and not the angle of insertion nor the zone of the gluteus muscle injected.  Dynamic cadaver studies demonstrate that fat placed just beneath the gluteus maximus fascia will migrate deep to the sub muscular space with the potential for venous tear and pulmonary fat embolism. A call is hereby made for a halt to fat grafting beneath the fascia of the gluteus muscle.

In this video the MACS-lift is presented as the cornerstone of our current concept of comprehensive facial rejuvenation. It effectively and safely treats the aging of the lower half of the face and the upper neck in the majority of aging men and women. The addition of microfat grafting to treat the age-related centrofacial deflation adds to the power and longevity of the rejuvenation, and also allows treatment of non-age-related features such as the tear trough and the nasolabial fold. To treat the category of patients with a more demanding neck laxity, platysmal bands and submental fat deposition we can add a subplatysmal fat resection via a submental incision, and/or a posterior subplatysmal neck plasty with posterior platysma suspension. All of these techniques are included in detail in this video, together with some theoretical considerations.

Part 1 of 3. This video demonstrates a circumferential truncal rejuvenation procedure by performing by incorporating a full abdominoplasty with excision of sub-Scarpal fat, concurrently with circumferential SAFELipo. The three-step process of SAFELipo allows complete, circumferential liposuction of the entire trunk including the area of the abdominoplasty flap, without disrupting the vascular supply of the flap. Excision of the sub-Scarpal fat of the abdominoplasty flap eliminates the parasitic deep fat, decreasing the metabolic load on the flap. These techniques allow for a circumferential rejuvenation of the entirety of the trunk while minimizing complications.

Part 2 (of 3) of a surgical video from the 46th Annual Baker Gordon Educational Symposium which features the concepts, techniques, and surgical results of Dr. Simeon Wall Jr’s SAFELipo® method of comprehensive body contouring. SAFELipo (Separation, Aspiration, and Fat Equalization) provides a nonthermal liposuction, fat-shifting, and fat-grafting method that aims to minimize bleeding, tissue trauma and contour deformities while maximizing smooth and even fat removal, and can be utilized with excisional procedures safely and effectively. SAFELipo is based on a three-step process. First, the fat globules are separated from one another and from their attachments such as blood vessels, nerves and the stromal architecture. The fat separation is performed utilizing specialized probes or basket cannulas, with or without a power-assisted vibrational device, leaving separated and viable fat behind. Next, aspiration of the separated fat is performed with less aggressive cannulas. This step minimizes suction avulsion injuries to the vascular and stromal bed, as the already separated and largely intact fat cells are preferentially harvested. This is followed up with the final step of fat equalization, where the remaining bed of fatty tissues is smoothed out and aesthetically contoured, without any further aspiration. Fat equalization leaves behind a smooth subcutaneous layer of “local fat grafts” that prevent adhesion of dermis to fascia, minimizing the risk of resultant contour deformities otherwise seen with dramatic and/or superficial liposuction.

Lipoabdominoplasty represents a change of traditional concepts about undermining. It can be performed through limited undermining, by using a liposuction cannula to move the flap down.

Before this new concept, it wasn’t safe to perform lipoplasty and abdominoplasty at the same surgical time.

In this lecture we can see that now it is quite safe to perform them together and even get better results because this approach preserves almost all perforating vessels of the abdomen, branches of the deep epigastrics – superior and inferior ones.

Also it maintains all abdominal anatomic structures, including the Scarpa fascia, which has a fundamental importance to decrease the postoperative edema, because the lymphatic vessels are preserved.

Finally, it results in fewer complications and surgical revisions.
