In this video the MACS-lift is presented as the cornerstone of our current concept of comprehensive facial rejuvenation. It effectively and safely treats the aging of the lower half of the face and the upper neck in the majority of aging men and women. The addition of microfat grafting to treat the age-related centrofacial deflation adds to the power and longevity of the rejuvenation, and also allows treatment of non-age-related features such as the tear trough and the nasolabial fold. To treat the category of patients with a more demanding neck laxity, platysmal bands and submental fat deposition we can add a subplatysmal fat resection via a submental incision, and/or a posterior subplatysmal neck plasty with posterior platysma suspension. All of these techniques are included in detail in this video, together with some theoretical considerations.

This presentation focuses first on the ability to create a tissue pocket in a precise manner with the aid of endoscopic assistance. This allows for the transaxillary approach to be used for breast augmentation in patients with many different tissue types, including patients with minimal ptosis. Additional refinements are then shown which also allow for the use of shaped highly cohesive textured surface implants to be placed using the transaxillary endoscopic approach. Lipoabdominoplasty represents a change of traditional concepts about undermining. It can be performed through limited undermining, by using a liposuction cannula to move the flap down.

Before this new concept, it wasn’t safe to perform lipoplasty and abdominoplasty at the same surgical time.

In this lecture we can see that now it is quite safe to perform them together and even get better results because this approach preserves almost all perforating vessels of the abdomen, branches of the deep epigastrics – superior and inferior ones.

Also it maintains all abdominal anatomic structures, including the Scarpa fascia, which has a fundamental importance to decrease the postoperative edema, because the lymphatic vessels are preserved.

Finally, it results in fewer complications and surgical revisions.

This patient had two pregnancies and two children. She presents a medium abdominal flaccidity, no stretch marks, central abdominal lipodistrophy and diastasis. She had breast augmentation prothesis that were changed at the same surgical time as lipoabdominoplasty.
