The PAP flap can also be harvested in a transverse orientation; however, this requires the presence of a proximal perforator which is not always present.

This video demonstrates a patient with moderate ptosis and an upper outer quadrant breast cancer. She did not have macromastia and wished to undergo breast preservation. The tumor that was resected through the standard mastopexy markings with radiographic confirmation of the clip and tumor within the specimen. The cavity was sampled and clipped.

The defect was reconstructed using an extended superomedial autoaugmentation technique. The extended pedicle was rotated up to fill the defect and the medial and lateral pillars were plicated. A contralateral symmetry procedure was performed removing more tissue to ensure symmetry in anticipation of some radiation fibrosis.

Most women with breast implants are satisfied with their results and their plastic surgeons. Recently, however, two high-profile issues have prompted increased concern: breast-implant associated anaplastic large cell lymphoma (ALCL) and breast implant illness.
