We report the case of a 69-year-old woman with ovarian metastasis as initial presentation. Ultrasonography revealed a multicystic mass in the right adnexa, which was considered a malignancy. FDG PET/CT performed to detect possible metastasis revealed a right adnexal mass with mild activity and an additional FDG focus in the tail of the pancreas, which later proved to be ovarian metastasis from pancreatic carcinoma.
