Here we present a previously unreported case of unexpected significant subdural hematoma after hybrid surgery for unruptured giant intracranial aneurysm with low-dose heparin administration. The detailed nature, pattern, and neuroradiologic findings of this subdural hematoma after hybrid surgery are discussed here along with review of the related literature.
