The dermoscopic features of melanocytic nevi in children are clearly different from those in adults. While nevi in children show almost exclusively globular patterns, those in adults usually exhibit reticular patterns. These remarkable differences could be explained by a dual concept of nevogenesis postulating a primary dermal origin of globular nevi and a primary epidermal origin of reticular nevi. Recent studies using confocal reflectance microscopy or histopathology support this dual pathway of nevogenesis and challenge the established paradigms of downward or upward migration of melanocytes during nevus evolution. Finally The new understanding of the origin and evolution of nevi lead also to new concepts regarding melanoma-genesis.
