These authors did try to produce Class I research, but that is very, very difficult in surgical patients. Seventy-three percent were randomized, but 27% refused. It is fine to include these patients in the study, and Class II research is respectable. More important is the need to have either independent (blinded) assessment or graded, self-reported patient response from which we can draw conclusions.
