The authors thank the Robert Wood Johnson Foundation's Health Care Financing and Organization initiative for its support. They appreciate the opportunity to build on the local health department profile data collected by the National Association of County and City Health Officials, and they thank Mary Watson of Social & Scientific Systems and the data collection team at Social & Scientific Systems for their excellent work. Finally, the authors could not have conducted the study without the participation of many dedicated public health professionals who completed their survey, and they are immensely appreciative of their time and effort.
