This case report describes a rare gynecologic complication from a pancreas-kidney transplant. This is a unique presentation of vulvar tissue autodigestion and edema that significantly contributes to the understanding of possible female lower genital tract injury after transplantation. No previous instances of vulvar injury have been reported in the literature, and there is a dearth of published information on these complications with respect to the female lower genital tract. This journal readership will be most likely to encounter this complication, and awareness may prevent significant patient morbidity and delay of diagnosis.
