A silicone 3-piece posterior chamber IOL, which had been held in place by optic entrapment in the capsulorhexis, became dislocated laterally after blunt trauma. The patient now has symptoms, and the situation must be repaired.

I see little alternative to refixating the optic by entrapping it in the capsulorhexis again (warning the patient to wear safety glasses) or removing the lens and replacing it with an Alcon Kelman Multiflex II AC IOL, making sure the lens is sized properly by performing intraoperative gonioscopy.
