I would perform phacotrabeculectomy with a triangular scleral flap in the superior position, use 2 iris retractors superiorly, 2 viscoelastic agents with the soft-shell technique, and implant 1 silicone plate IOL at 180 degrees. At the end of surgery, depending on the mydriasis, I would consider an iris suture inferiorly.
