Christiana Care Visiting Nurse Association (VNA) developed a fall risk assessment using information obtained from the Outcomes and Assessment Information Set (OASIS) assessment. The fall risk assessment tool has been tested using a retrospective record review. A risk-specific intervention guide and postfall algorithm also are parts of the Fall Prevention Program. The Fall Prevention Program has improved patient safety and reduced emergent care related to injury from a fall.
Gale Moore Bucher, RN, MSN, is a Clinical Nurse Specialist and the Performance Management Coordinator at Christiana Care VNA, New Castle, Delaware. She is also a member of the Fall Prevention Team at Christiana Care Health System, Adjunct Instructor for the University of Delaware's College of Health and Nursing Sciences, and survey team member for the Commission of Collegiate Nursing Education.
Pamela Szczerba, PT, MPT, serves as the statewide Rehab Consultant for Christiana Care VNA, as a member of the Fall Prevention Team, and also as an Adjunct Instructor for the University of Delaware Physical Therapy Program, New Castle, Delaware. She is also a consultant for OASIS Answers, Inc., Redmond, WA.
Patricia M. Curtin, MD, FACP, CMD, is a board-certified geriatrician and internist and the Section Chief of Geriatric Medicine at Christiana Care Health System, the Medical Director of two nursing homes, the Vice-Chair of the VNA Board of Directors and the medical Director of the WISH (We Improve Senior Health) Program and ACE (Acute Care of the Elderly) Unit at Christiana Care Health System, Wilmington, Delaware.
