ERRATUM

In the print version of the Summer 2018 issue of Frontiers of Health Services Management (vol. 34, no. 4), in the article by Maxine dellaBadia Simon, FACHE, “Compliance and High Reliability in a Complex Healthcare Organization,” Exhibit 2 on page 25 was missing the “Y” in “HIGH RELIABILITY” in the center of the image, and the legend text in the bottom left corner of the image was cut off. These errors were introduced by the compositor during preparation of the print files; the online version of the exhibit was not affected. The corrected version of the exhibit appears below.
