EVIDENCE-BASED ANSWER Caffeine increases self-rated anxiety more than placebo, but the dose-response relationship is inconsistent (SOR: B, 2 small RCTs). This effect may be exclusive to men (SOR: C, small RCT). Patients with generalized anxiety disorder are more sensitive to the anxiogenic effects of caffeine than patients with panic disorder or no psychiatric illness (SOR: B, small RCT).
