On page 86, the first sentence of the introduction states:

“Despite its extraordinary implications, Kemp's discovery that the ear makes sound while listening to sound had difficulty getting published (reviewed in Kemp, 1978).”

The correct reference citation should be Kemp, 1998:

“Despite its extraordinary implications, Kemp's discovery that the ear makes sound while listening to sound had difficulty getting published (reviewed in Kemp, 1998).”

on page 87 was printed in black and white. The figure should have been published in color, as seen below:
