Monitoring of Free Valproic Acid Levels?

Valproic acid exhibits three unique pharmacological and pharmacokinetic characteristics: (a) a nonreversible pharmacologic effect, (b) a nonlinear level-dose relationship, and (c) a high and variable degree of plasma protein binding. These properties and their clinical implications are discussed and a case is made for monitoring free vs total plasma levels in specific circumstances.
