Stroke is recognized in less than 10% of organ transplant recipients but in autopsy studies is identified as leading or contributing to death in 40% to 60%. The spectrum of cerebrovascular complications is broad, including both ischemic and hemorrhagic stroke from multiple underlying mechanisms, and varies with the type of organ(s) transplanted, the time from transplantation, and associated medical comorbidities.
