In this article in the November 2009 issue (page 2919), an author name was spelled incorrectly. The correct spelling is Philippe Aegerter.

On page 2923, in column 1, lines 6–8, the following statement is now corrected:

Crude hospital mortality was 251 of 659 (38.1%) in referred patients and 466 of 1981 (23.5%) in nonreferred patients (p < .0001).

In the Appendix, three people in the Head Intensive Care Unit section (D. Benhamou, Y. Ozier, and D. Safran) should have been replaced with C. Richard, J. P. Mira, and J. Y. Fagon.
