A23-year-old otherwise healthy male had peripapillary staphyloma1 with optic disc anomaly in his left eye (Fig. 1A). Retina inside the staphyloma had a full-thickness break with detachment (Fig. 1B). Retina outside the staphyloma could be differentiated into temporal with pigmentary changes and nasal normal with sharp demarcation between the 2 clinically pointing toward a self-settled rhegmatogenous retinal detachment (Fig. 1A). There was no other peripheral break. We hypothesize there are 2 factors that could have helped settle the detachment/prevent progression: first, a well-formed vitreous; second, the staphyloma itself could have acted as a physiological encirclage separating retina into 2 compartments preventing subretinal fluid communication. The extra-staphylomatous part had a healthy retinal pigment epithelium to pump fluid out which the staphylomatous part lacked.
