The concept of the medical home, extended to surgical patients, has the potential to provide a robust mechanism for streamlining and coordinating care before, during, and after operative procedures. In its current state, this care can be fragmented, especially for patients with chronic pain conditions that may require medical tapering regimens following an acute surgical episode on top of baseline conditions. In this infographic, we review the scope of chronic postsurgical pain, the risk factors for its development, and the role that a perioperative pain service might play in perioperative care coordination for this complex patient population.
