Permission was granted to use the Amputee Body Image Scale (ABIS), a questionnaire devised by James Breakey, PhD, CP, president of Breakey Prosthetics, Inc., San Jose, California.
My research and manuscript included the Amputee Body Image Scale (ABIS), a questionnaire devised by James Breakey, PhD, CP, which he developed as part of one of his research studies. Dr. Breakey is President of Breakey Prosthetics, Inc., in San Jose, CA. In developing my own project, I contacted Dr. Breakey and received his verbal permission to use the questionnaire for my own efforts. This verbal permission was received in April 2000 via a telephone call. Dr. Breakey is aware that the ABIS was used in my research and that it is included in the manuscript that I have submitted for publication.
