Total ankle replacement in valgus unstable ankles is challenging because misalignment of the ankle, especially in combination with valgus instability, has been identified as a major deteriorating factor for the result of total ankle replacement. This article describes a treatment algorithm based on the classification of valgus unstable osteoarthritic ankles. Three types of valgus unstable ankles are differentiated with regard to the treatment.
