Some studies suggest that a majority of the United States population may be ready for unsedated endoscopy. There is no data indicating whether nurses or physicians are willing to undergo endoscopy without sedation. We studied adult patients presenting for outpatient endoscopy at two large hospitals, as well as a random convenience sample of physicians and nurses. We found that few patients and even fewer physicians are willing to undergo unsedated endoscopy.

Seven patients with metachronous neoplasms of both lung and pancreas are reviewed. The spectrum of diseases encountered includes the combination of dual primary neoplasms, metastatic cancer originating in lung or pancreas, and metastatic cancer originating elsewhere. Immunohistochemical cytokeratin 20 staining aided in the differential diagnosis.

Most aeromedical obstetrical transports in the United States are performed by helicopters over relatively short distances. The authors describe a program which performs a large number of long-distance obstetrical transfers, and retrospectively review their experience with long-distance fixed-wing aeromedical transport of obstetrical patients in order to identify the complications encountered during transport.

Few migraine prophylactic therapies have demonstrated a 50% reduction in headaches. Even when successful, the economic burden of prophylaxis can discourage widespread usage. This article presents a pilot study of a novel, effective, specific, and inexpensive prophylactic strategy for menstrual-associated migraine.

The past three decades have seen a dramatic increase in the diagnosis of Barrett's esophagus and esophageal adenocarcinoma in the United States. Photodynamic therapy, a recently approved, minimally invasive endoscopic ablation technique, has recently been approved as an alternative to esophagectomy. This paper describes the authors' experience using endoscopic mucosal resection before photodynamic therapy in patients with Barrett's high-rate dysplasia and adenocarcinoma.
