A 93-year-old woman with neovascular age-related macular degeneration was treated with photodynamic therapy with stabilization of choroidal neovascularization for 3 years. Two days after spectral domain optical coherence tomography failed to detect any intraretinal or subretinal fluid, the patient returned with a large submacular hemorrhage and 5/400 vision. Controversies regarding criteria for retreatment and the “treat and extend” approach are discussed.
