Thomas Baker is a visionary who is recognized not only as an innovator of techniques for facial rejuvenation and chemical peeling but is also revered for his contributions as an educator who brought credibility to the subspecialty of aesthetic surgery. This lecture focuses on Dr Baker's academic accomplishments as well as celebrating his foresight in forming, in 1967, the first plastic surgery educational symposium to focus on aesthetic surgery, incorporating live surgery demonstrations as an educational tool to improve the practice of cosmetic surgery procedures globally.

This presentation focuses first on the ability to create a tissue pocket in a precise manner with the aid of endoscopic assistance. This allows for the transaxillary approach to be used for breast augmentation in patients with many different tissue types, including patients with minimal ptosis. Additional refinements are then shown which also allow for the use of shaped highly cohesive textured surface implants to be placed using the transaxillary endoscopic approach. The combination of technical nuances presented shows that the transaxillary endoscopic approach can be used for placement of smooth and textured silicone gel breast implants in patients of all tissue types who are otherwise good candidate for breast augmentation.

This video discusses and introduces the finesse needed to reshape the midface using neuromodulators and several different types of hyaluronic acid fillers with a sharp versus a blunt cannula. A detailed analysis of the face is performed before each injection of both neuromodulator and hyaluronic acid filers. Based on this analysis and the patient’s desires and requests, the process is begun with the neuromodulators from the forehead, glabella, and crow’s feet areas to the chin, depressor anguli oris muscles, and platysmal muscles in the neck areas. The hyaluronic acid fillers are used to fill and reshape the malar (cheek) and perioral areas as well as the tear trough areas in a manner to soften and highlight these areas to provide a more natural youthful face. To address these concerns, the patient underwent removal of her subglandular implants, reattachment of the pectoral muscle to the overlying breast gland and insertion of a new pair of silicone gel implants in the subpectoral plane. Concomitantly, bilateral circumareolar mastopexies were performed using the periareolar pinwheel technique.

Part 1 of 2. This video demonstrates Dr. Rohrich’s Upper Lid Blepharoplasty and the innovative Five-Step Lower Lid Blepharoplasty eyelid-cheek blending technique. Lower lid blepharoplasty has potential for major long-lasting complications and marginal cosmetic outcomes if performed incorrectly, or if the anatomical aspects of the orbicularis oculi are disregarded. This has detracted many surgeons from performing the technical maneuvers necessary for optimal periorbital rejuevenation which blends the eyelid-cheek junction. A simplified five-step clinical approach based on sound anatomical principles is presented in the video. Aesthetic outcomes in lower lid blepharoplasty can be improved using this five-step technical sequence. The five steps which will be discussed and illustrated by this live surgery include: (1) augmentation of the supportive deep malar fat compartment; (2) preservation of the lower lid orbicularis oculi muscle with minimal transconjunctival fat removal (if at all); (3) selective release of the the main retaining structure (orbicularis retaining ligament); (4) establishment or strengthening of the lateral canthal support (lateral retinacular suspension); and (5) minimal skin removal using a skin pinch technique.
