Foundation of Technology Builds Successful Patient-Centered Medical Home in Western New York State

The National Committee for Quality Assurance (NCQA) defines the patient-centered medical home (PCMH) as “a way of organizing primary care into ‘what patients want it to be.’” There are numerous benefits to this model of care, NCQA says, including higher quality and lower costs. Medical homes also can “improve patients’ and providers’ experience of care.”
