Management of a Tracheo-Gastric Fistula After Esophagectomy With a Polyflex Stent

Abstract

We report the placement of a silicone-coated polyester stent (Polyflex; Boston Scientific-Watertown, MA) for long-term control of a tracheo-gastric conduit fistula in a 75-year-old man who underwent tri-incisional esophagectomy for T1N0 adenocarcinoma in the setting of Barrett esophagus with high-grade dysplasia. The stent was placed in the airway with good coverage of the fistula, which was located 3 cm above the carina. The stent had to be exchanged once for acid/bile-induced damage despite the use of proton pump inhibitors. This report is the first to describe the long-term use of a polyester stent to control a tracheo-gastric conduit fistula long-term and describing the potential need for stent change secondary to acid/bile-induced damage.
